NAIL CARE
You will benefit from foot care if:
You have limited ability to reach your feet
You have vision problems that make it difficult for you to see your feet well enough to perform your own foot care
You have limited strength or agility in your hands
You have thickened nails that are difficult to trim
You have toe nail fungus
You rely on others to trim your nails who are often unavailable
You are diabetic and therefore at a higher risk of developing foot problems due to decreased sensation to your feet
You have poor circulation to your feet

PILL PLANNER MANAGEMENT
Set up and maintain pill planner
Educate patient and patient's family on proper dosage, frequency and side effects
Evaluate therapeutic response to medications
Assess the effectiveness of pain management
Coordinate with patient's physician as needed
Assess for duplicate medications
Assist in developing medication schedule/process to enhance compliance
Review medication regime (complete listing of all prescribed and over-the-counter medicines)

CARE MANAGEMENT
What is a Care Manager?
​
A care manager is usually a licensed nurse or social worker who has experience in geriatrics and an extensive understanding of local healthcare resources.
Your Care Manager can help find resources to make your daily life easier. She will work with you to form a long-term care plan and assist in finding you the services you need.
Care managers can be especially helpful when family members live far away.
